Walk-in Enrollment Requirements & Process
New Students (First Year)
Requirements:
- SHS 10- Original Copy
- Certificate of good moral character (original copy)
- Two (2) photocopies of the authenticated PSA birth certificate (attach the original copy for validation)
- Marriage contract (for female only)
- Passport size I.D. picture with white background and with complete name tag (2 copies)
- ALS Certification, Diploma in Secondary Education Equivalency if enrolled in Alternative System Accreditation (ALS)
Process:
Step 1:
- Go to the Admissions/Scholar Officer
- Submit credentials: Form 137-A, Certificate of Good Moral Character, PSA Birth Certificate, and other pertinent documents needed for enrollment
- Fill up the forms and waivers
Step 2:
- Go to the Guidance Office
Step 3:
- Go to the Program Head
- Evaluation of subjects to be enrolled
- Interview
Step 4:
- Go to AGH for a physical exam/laboratory test requirements
Step 5:
- Pay the downpayment at the Cashier’s office
- Present your acknowledgement slip from AGH
- Get a pre-enrollment form
- Fill-up the “No Refund Policy” waiver
Step 6:
- Go to Subject Schedule
- Enroll your subjects according to the semester of your curriculum year
- Fill-up the pre-enrollment form and copy the subjects correctly
Step 7:
- Proceed to your program Head/Department Dean
- Approval of subjects to be enrolled
- Fill-up the forms and waivers
Step 8:
- Go to processing area (Registrar)
- Present your duly accomplished pre-enrollment form for processing
- After validating and encoding of schedule, be sure TO SIGN and claim your official white form & student handbook at the designated issuance area.
Old/Continuing Students
- Requirements:
- Grades from last semester
- Validated Evaluation sheets from the Department Head
- Physical examination results from AGH
Returnees / Returning Students
- Requirements:
- Evaluation Sheet duly certified by the Registrar
- Passport size of I.D. picture with white background and white complete name on it
- Passed the pre-exam interview by the Dean
- Physical examination results from AGH
Process:
Step 1:
- Go to the Program Head
- Evaluation of academic status
Step 2:
- Go to AGH for a physical exam/laboratory test requirements
Step 3:
- Pay the downpayment at the Cashier’s office
- Present your acknowledgement slip from AGH
- Get a pre-enrollment form
Step 4:
- Go to Subject Schedule
- Enroll your subjects according to the semester of your curriculum year
- Fill-up the pre-enrollment form and copy the subjects correctly
Step 5:
- Proceed to your program Head/Department Dean
- Approval of subjects to be enrolled
- Fill-up the forms and waivers
Step 6:

Transferees
Requirements:
- Certificate of Transfer Credentials (Honorable Dismissal)
- Transcript of Records (for evaluation purposes only)

Scholarship Programs
MMFC provides the following in-house scholarships, and implements as well scholarship programs of government and non government agencies.
Academic Scholarship Program
- This grant is good for the entire duration of the chosen course with the following specific provisions:
- A full academic scholar enjoying a 100% tuition fee discount should maintain a general weighted average of 95% or better, with no grade lower than 90%.
- A semi-full academic scholar enjoying a 75% tuition fee discount should maintain a general weighted average of 90% or better, with no grade lower than 85%.
- A half academic scholar enjoying a 50% tuition fee discount should maintain a general weighted average of 88% or better, with no grade lower than 85%.
- Every scholar should take a full academic load every semester as prescribed by the curriculum he/she has chosen.
- A full scholar who fails to maintain the required grade may be demoted to any of the lower ranks of scholars depending on the grade he/she have attained during the semester, or summer. This rule applies to all other scholars, however if the scholar achieves a higher weighted average grade within his/her bracket, he/she may be promoted to a higher level of scholarship.
Honor Student Scholarship Program
This scholarship program is given to all honor students (Valedictorian and Salutatorian only) of different public and private highschools.
For Valedictorian, a full discount on tuition fee and 50% discount on miscellaneous fees.
- He/she shall maintain a general weighted average of 95% or better, with no grade lower than 90% in a semester.
- He/she shall be required to participate in curricular and co-curricular activities authorized by OSA and Scholarship Coordinator.
- He/she shall pass the minimum standards set forth by the various programs offered by the College.
- He/she shall abide by the rules and regulations of the scholarship program of the school, particularly the grade requirement.
For Salutatorian, a full discount on tuition fee and 25% discount on miscellaneous fees.
- He/she shall maintain a general weighted average of 92% to 94%, and with no grade below 90% in a semester.
- He/she shall be required to participate in curricular and co-curricular activities authorized by OSA and Scholarship Coordinator.
- He/she shall pass the minimum standards set forth by the various programs offered by the College.
- He/she shall abide by the rules and regulations of the scholarship program of the school, particularly the grade requirement.
Community Tulong-Aral Scholarship Program
This scholarship granted to all High School students is determined by the strong and quick response of the student, as an invitation for the Scholarship Program of the institution with the specific provisions:
- For Nursing, Medical Technology, Pharmacy, Midwifery, and Radiologic Technology students, he/she shall maintain a general average of 85% or better, with no grade below 80% in a semester.
- For Dentistry students, he/she shall maintain a general average of 87% or better, with no grade below 82% in a semester.
- He/she shall be required to participate in curricular and co-curricular activities authorized by OSA and Scholarship Coordinator.
- He/she shall pass the minimum standards set forth by the various programs offered by the College.
- He/she shall abide by the rules and regulations of the scholarship program of the school, particularly the grade requirement.
Physical Therapy and Optometry Scholarship Program
This scholarship program is offered to all High School Graduates who will be taking up a Physical Therapy or Optometry Program. This grant will give the student the opportunity to enjoy a full tuition fee credit with the specific provisions:
- He/she shall maintain a grade not lower than 82% in all subjects enrolled in a semester.
- He/she shall maintain a general weighted average not lower than 87% or its equivalent in a semester.
- He/she shall be required to participate in curricular and co-curricular activities authorized by OSA and Scholarship Coordinator.
- He/she shall pass the minimum standards set forth by the various programs offered by the College.
- He/she shall abide by the rules and regulations of the scholarship program of the school, particularly the grade requirement.
Community Tulong-Aral Scholarship Program
This scholarship granted to all High School students is determined by the strong and quick response of the student, as an invitation for the Scholarship Program of the institution with the specific provisions:
